Fall Sports Singups

We are very excited to announce the St. John the Baptist DHS Athletic Department has partnered with FinalForms, an online forms and data management service. All parents/guardians and athletes will need to create an account and sign up for sports through FinalForms. The links below will help you navigate this simple process. From start to finish, it should take about ten minutes to register, and you will only have to fill out most of the forms once. You may review your data at any time to verify it is current. Forms will need to be verified prior to each sport season. Sport physicals MUST continue to be brought or sent to the building nurse for final clearance.
